Membership Eligibility
Membership in CareConnections Federal Credit Union is open to the employees of:

  • Catholic Healthcare West - Southern California Region
  • California Hospital Medical Center
  • Glendale Memorial Hospital and Health Center
  • Northridge Hospital Medical Center
  • San Gabriel Valley Medical Center
  • Valley Presbyterian Hospital
  • Southern California Orthopedics Institute
  • PacifiCare Health Systems
  • Arroyo Verde Medical Group
  • Facey Medical Group
  • Physican Associates of the Greater San Gabriel Valley
  • Trinity Care
  • VNA Care, Inc.
  • Astoria/G & E Healthcare, Inc.

  • Regency Park Westwood, Inc.

  • Members' immediate family as defined below:
    1. Immediate family is limited to spouse, child, sibling, parent, grandparent, and grandchild if not living in the same residence. Step- and adopted children, parents, grandparents, grandchildren, siblings, are included in this definition.
    2. Domestic partner including permanent member of the same residence who participates in the maintenance of the household. Proof of residence such as lease or rental agreement, mortgage documents, or utility bills may be required.

Membership eligibility is on the first day of employment. Minimum deposit for active membership is $20, plus $5 non-refundable membership fee. Members who leave their employment may keep their membership and account open.

Once a Member, Always a Member

Potential members must obtain a Tax Identification Number (TIN) before applying for credit union membership.

Interested in CCFCU membership? All you need to do is complete our new accounts forms, which you can download here.

Instructions
To enroll into the credit union, complete and sign the membership application and appropriate form (W-9 or W-8). Send original forms with the following:

  1. $25 check or money order payable to CCFCU (please do not send cash).
  2. Clear copy of applicable identification documents (i.e., Driver's License/ID Cards or Passport/Alien Identification Card, etc.)
  3. Clear copy of your last paystub to validate your eligibility.

Important Notice

What this means to you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.
